Treatment of acute toothache and injuries in Prague

What do we consider to be an emergency requiring immediate treatment?
Acute dental conditions can manifest themselves in various ways, but generally include situations that cause severe pain, bleeding, swelling, or threaten permanent damage to the tooth or surrounding tissues. The most common include:
- Intense, throbbing tooth pain: Often a sign of deep decay affecting the pulp (pulpitis) or inflammation around the root of the tooth (periodontitis, abscess). The pain may be spontaneous, worsen at night, when biting, or in response to hot or cold stimuli.
- Swelling of the face, gums, or lymph nodes: This may signal a spreading infection (abscess) that requires immediate intervention to prevent more serious complications.
- Tooth injuries: Broken tooth (crown or root fracture), cracked tooth, loose tooth (subluxation), displaced tooth (luxation), or completely knocked-out tooth (avulsion). The speed of treatment often determines whether the tooth can be saved.
- Severe bleeding from the mouth: After tooth extraction, injury, or spontaneous bleeding from the gums that cannot be stopped by normal means.
- Loss or damage to a filling, crown, or bridge: If it causes a sharp edge that irritates the tongue or cheek, or if the exposed tooth is painful.
- Pain and complications during wisdom tooth eruption: Inflammation of the gums around the erupting tooth (pericoronitis), which can be very painful and accompanied by swelling and difficulty opening the mouth.
If you are unsure whether your condition requires immediate treatment, please do not hesitate to contact us. Even a telephone consultation can help assess the situation and recommend further action.
What to do in case of acute pain or injury?
For severe toothache:
- Rinse your mouth with lukewarm water or a mild saltwater solution.
- Carefully use dental floss to remove any food debris between your teeth.
- Do not apply aspirin directly to the painful tooth or gum, as it can burn the tissue. Take commonly available analgesics (painkillers) if you are not allergic to them, but remember that this is only a temporary solution.
- You can apply a cold compress (ice wrapped in a towel) to the outside of your face to reduce any swelling.
- Contact our Dobrý Dent clinic as soon as possible to arrange an appointment.
- In case of tooth injury (breakage, knock-out):
- Broken tooth: Find the broken piece, if possible, and bring it with you. Rinse your mouth with lukewarm water. Contact us immediately.
- Knocked-out tooth: This is a real emergency! Find the tooth, hold it only by the crown (the part visible in the mouth), never by the root. Rinse it carefully under running cold water (max. 10 seconds), do not rub it. Try to gently put the tooth back into the gum socket and hold it in place by biting down on a tissue or gauze. If this is not possible, place the tooth in a small container with milk, saline solution, or, in the worst case, place it in your mouth between your cheek and gum (be careful not to swallow it). Seek our help immediately – time is critical in saving the tooth!
- Loose or displaced tooth: Try to manipulate the tooth as little as possible. Do not bite on it. Visit our clinic as soon as possible.
- Soft tissue injury (tongue, lips, cheeks): Carefully clean the affected area. Apply pressure with clean gauze or cloth to stop the bleeding. A cold compress can help reduce swelling and pain. If the cut is deep or the bleeding cannot be stopped, seek medical attention (dental or general emergency room).
How are acute conditions treated at the Dobrý Dent clinic?
Our primary goal is to relieve your pain as quickly as possible and stabilize the situation. Thanks to our modern equipment and experience, we can offer comprehensive care:
1. Diagnostics – the basis for successful treatment
We use the latest diagnostic technologies to correctly determine the cause of your problems and plan the most appropriate treatment:
- Digital X-ray (RTG) and 3D CBCT: These imaging methods allow us to obtain a detailed image of the teeth, roots, jawbones, and surrounding structures. CBCT (Cone Beam Computed Tomography) is invaluable, especially in diagnosing hidden inflammation, root cracks, complicated cases for endodontics (root canal treatment), or when planning wisdom tooth extractions and assessing injuries. It provides us with a three-dimensional view that reveals problems invisible on a standard 2D X-ray.
- Intraoral camera and 3D scanner: These allow us and you to see the condition of your mouth in detail on the screen. In addition, the 3D intraoral scanner can quickly and comfortably create a digital impression of your teeth, which is useful, for example, when a temporary replacement is needed after an injury or when planning definitive treatment.
- Careful clinical examination: This includes a visual inspection, palpation, tooth vitality tests, and an assessment of your symptoms.
2. Pain relief – gentle and effective
We understand that fear of pain is often the main barrier to visiting the dentist, especially in acute cases. That is why we place great emphasis on painless treatment:
- Local anesthesia: We use highly effective anesthetics to ensure comfort during the procedure.
- Computer-controlled STA (Single Tooth Anesthesia): This system allows the anesthetic to be administered very slowly and in a controlled manner, minimizing the unpleasant sensation of injection and pressure. Anesthesia is often limited to the tooth being treated, without the unpleasant feeling of numbness in the lip or tongue. It is an ideal solution for patients who are afraid of injections.
3. Treatment itself – modern procedures for the best results
Based on the diagnosis, we select the most appropriate treatment procedure:
- Treatment of deep caries and pulpitis: Removal of caries and preparation of a high-quality filling. If the dental pulp is affected, we perform endodontic treatment (root canal treatment). At our clinic, we perform this treatment using a surgical microscope, which ensures maximum precision, thorough cleaning, and filling of the root canal system, which significantly increases the success of the treatment and the long-term prognosis of the tooth.
- Abscess treatment: Ensuring drainage of pus (incision), cleaning and disinfecting the affected area, and administering antibiotics if necessary. This is followed by treatment of the causative tooth (most often endodontic treatment or extraction).
- Treatment of injuries:
- Crown fractures: Reconstruction of the tooth with composite material or fabrication of a crown.
- Complicated fractures (affecting the pulp): Endodontic treatment and subsequent reconstruction.
- Loose or displaced teeth: Repositioning (returning to the correct position) and splinting (fixation to adjacent teeth) for the time necessary for healing.
- Knocked-out teeth: Replantation (reinsertion into the socket) and splinting, if performed in a timely manner and under the right conditions.
- Root fractures: The prognosis depends on the location and type of fracture; extraction is sometimes necessary.
- Tooth extraction: If the tooth cannot be saved, we will perform the extraction as gently as possible. We will then discuss tooth replacement options with you (implant, bridge).
- Treatment of pericoronitis: Rinsing, disinfection, or removal of the part of the gum covering the tooth, or extraction of the wisdom tooth.

Don't put off solving urgent problems
Acute toothache or injury to the mouth is not only unpleasant, but can also lead to more serious health complications if not treated in time. The infection can spread, the damage to the tooth can become irreversible, and the chances of saving an knocked-out tooth decrease with every passing minute.
At Dobrý Dent, we are ready to help you. We understand the urgency of your situation and strive to offer patients with acute problems the earliest possible appointment. Our goal is not only to relieve pain and treat damage, but also to restore the function and aesthetics of your smile using the best available procedures and materials.
