All-on-4 Implants: Restore Confidence and Function to Your Smile

The All-on-4 concept (sometimes referred to as „all on four“) is an innovative and scientifically proven procedure for complete dental reconstruction in an edentulous jaw or a jaw with no longer salvageable teeth. Its essence lies in anchoring a fixed (non-removable) bridge, which replaces all the teeth in the jaw, to just four strategically placed dental implants.

Two implants are inserted perpendicularly in the front of the jaw, while the two posterior implants are inserted at a specific angle (usually 30-45 degrees). This unique oblique placement of posterior implants has several key advantages:

• Maximizes the use of existing bone: It allows you to bypass anatomically sensitive areas (such as the maxillary sinuses in the upper jaw or the nerve canal in the lower jaw) and utilize the higher-quality bone in the front of the jaw.
• Often eliminates the need for augmentation: Thanks to oblique placement, in most cases, it is possible to avoid complex and time-consuming bone augmentation procedures (augmentation, sinus lift), which are often necessary with traditional implantation methods when bone is lacking.
• Ensures excellent stability: The distribution of forces over four implants provides a solid foundation for immediate or very early loading with a fixed bridge.

The implants are made of biocompatible titanium, a material that is very well accepted by the human body and which allows osseointegration – a solid connection of the implant with the bone. A highly aesthetic and fully functional bridge that looks and functions like your own teeth is then attached to these implants.

The All-on-4 method is an ideal solution for patients who:

• Are completely edentulous in one or both jaws.
• Have most of their teeth in very poor condition (advanced periodontitis, extensive caries, fractures) and their rescue is no longer possible or meaningful.
• Wear removable dentures and are dissatisfied with their stability, function or comfort.

1. Consultation and precise planning

The first step is a non-binding consultation, during which we will discuss your wishes, concerns and health history. This is followed by a comprehensive examination of the oral cavity. A key part of the planning is the use of our 3D CBCT X-ray. This device provides us with a detailed three-dimensional image of your jawbone, which allows us to:

• Accurately assess the quantity and quality of bone.
• Identify the position of important anatomical structures (nerves, blood vessels, cavities).
• Virtually plan the optimal position, length and inclination of each implant with millimeter accuracy.
• Minimize risks and ensure a predictable result.

We then use a 3D intraoral scanner, which replaces the unpleasant traditional impressions. This gives us a digital model of your jawbone, which serves as the basis for the design of a future fixed bridge and, if necessary, for the production of a surgical template, which further increases the accuracy of implant placement.

2. Surgical phase: Implant placement

The surgical procedure itself takes place under local anesthesia. At Dobrý Dent, we use the computer-controlled anesthesia STA (Single Tooth Anesthesia System) system, which ensures painless and comfortable administration of anesthetic without the unpleasant feeling of stiffness of the lip or cheek. If there are unsuitable teeth in the jaw, they are first gently removed.

Four titanium implants are then inserted according to a precise plan. Thanks to detailed 3D planning and the possible use of a surgical template, the procedure is quick and minimally invasive. In some cases, our surgeon may also use an operating microscope for maximum visualization and accuracy, especially in more complex situations. The entire procedure for one jaw usually takes several hours.

3. Immediate loading phase: „Teeth in one day“

One of the greatest advantages of the All-on-4 concept is the possibility of so-called immediate loading. In most cases, we will fit a temporary, but firmly screwed-in bridge on the day of surgery, or very soon after. This bridge is made in advance based on digital planning. You will leave immediately with functional and aesthetically acceptable teeth. You can eat (softer food in the initial healing phase) and smile without restrictions. You will wear this temporary bridge while the implants heal (usually 3-6 months).

4. Final prosthetic phase

After the implants have successfully healed into the bone (osseointegration), which we verify with control X-rays and stability tests, it is time to make the definitive bridge. Again, we use digital scanning for maximum accuracy. The definitive bridge is made of high-quality, durable and aesthetic materials (e.g. metal-ceramic, zirconia ceramic, special resin reinforced with a structure). This bridge is designed to perfectly match the shape of your jaw, the color of your natural teeth (if any remain as a reference), and your aesthetic preferences. The result is a permanent, strong, and beautiful dental replacement that will serve you for many years.
